NovaMed Medical Clinic is accepting new patients. Everyone joining the clinic completes a new patient intake package, which gives your physician your health history before your first visit.
The intake package is required. Anyone who would like to become a patient of NovaMed Medical Clinic must complete the appropriate intake form before being accepted as a new patient.
Which package to complete
Choose the one that matches the patient's age. A parent or guardian completes the pediatric package on the child's behalf.
- Adult package, for patients 18 and over. Six pages.
- Pediatric package, for patients under 18. Five pages, completed by a parent or guardian.

If you are not able to print the package, ask at the clinic and we will give you a paper copy.
What the package covers
Both packages ask about your health history, medications and allergies, previous care, and the health concerns you would most like your provider to know about. Both end with the clinic's policies and the consent choices you are asked to make.
How to return your completed package
Bring the completed package to your first appointment, or drop it off at the clinic during opening hours.
Please do not email your completed package. It contains personal health information, and email is not a secure way to send it. Our email address is for administrative enquiries only.
Booking your first appointment
Online booking for a family doctor appointment is for patients already registered with a provider at the clinic. If you are new to NovaMed, please call us at 236 429 6505 to register, or book a walk-in visit in the meantime.
